The case for and against

Updated Oct 21, 2026

Bull case

Achieve Life Sciences is attractive because cytisinicline appears to be a de-risked, differentiated therapy with robust phase 3 smoking cessation data, a positive phase 2 vaping cessation signal, and no FDA-identified clinical efficacy or safety deficiencies in the CRL. The company’s regulatory path now centers on manufacturing and labeling, with a 4Q26 NDA resubmission and potential 1H27 approval/launch, while a new manufacturing partner, fresh capital, and experienced leadership reduce execution risk. Fundamentally, the opportunity is large because smoking cessation remains underpenetrated with millions of failed quit attempts and about 10MM scripts annually, and cytisinicline’s better safety/tolerability versus existing options could support meaningful share gains and substantial commercial upside.

Bear case

Achieve Life Sciences is a highly speculative biotechnology company whose investment case is dominated by clinical, regulatory, and commercial uncertainty, with both smoking and vaping cessation programs vulnerable to trial failure, safety setbacks, or delayed FDA approval. Even with cytisinicline showing promise and an FDA end-of-Phase 2 meeting for vaping, the business still faces major commercialization hurdles from competition and insurer pressure, while a dilutive equity offering remains a material financing risk. The market opportunity is large, with about 25MM US adults still smoking, roughly 15MM annual quit attempts, and about 18MM vapers, but the company’s dependence on a single asset and uncertain partner needs make the downside case compelling.
