The case for and against

Updated Oct 21, 2026

Bull case

Bioventus is viewed favorably because its core franchises in hyaluronic acid injections, bone graft substitutes, and fracture care appear durable, with Durolane maintaining leadership and BGS expected to grow faster than low-single-digit market rates. The company has also demonstrated operational resilience, lifting adjusted EBITDA margins from the low teens in 2022 to over 20% in 2025 while generating over $72 million of free cash flow in 2025 versus less than $38 million in 2024. That cash is funding a targeted push into higher-growth areas—PNS, PRP, ultrasonics, and international—while debt reduction, a $600 million to $610 million revenue guide, and manageable leverage create optionality for future buybacks or M&A.

Bear case

Bioventus is exposed to structurally fragile demand because its products depend heavily on reimbursement, and the recent bone growth stimulation reimbursement cut could trim FY/26 revenue by about $2-3 million even after management reaffirmed guidance. The company also faces costly FDA and global regulatory hurdles, where delays or remediation can quickly pressure operating results in a highly competitive market with large, well-capitalized rivals across spinal fusion, motion preservation, and bone growth therapies. Its acquisition-led strategy adds integration, execution, and potential financing or dilution risk, while the need to recruit and retain sales representatives and distributors raises the chance of slower commercial traction and weaker financial condition.
