The case for and against

Updated Oct 21, 2026

Bull case

Capricor Therapeutics is viewed favorably because deramiocel has a credible late-stage catalyst profile, with the FDA accepting the major BLA amendment and extending the PDUFA to November 22nd, while open-label extension data and the HOPE-3 Lancet publication reinforce the case for upper-limb benefit in DMD. The investment case is further strengthened by measurable clinical evidence, including a 4.55% treatment difference in PUL 2.0 versus placebo at 12 months and a supportive 24-month dataset, which may improve approval odds despite the negative CTGTAC vote on cardiomyopathy. Financially, the company’s $238 million of cash at 2Q:26, potential warrant proceeds of $19.1 million, and an estimated $797 million 2030E U.S. net revenue share underpin a large asymmetric opportunity that could re-rate the stock.

Bear case

Capricor Therapeutics is a clinical-stage biotech with a highly concentrated thesis on deramiocel, and the data remain vulnerable to interpretation because the placebo crossover benefit in HOPE-2-OLE could be confounded by dropouts, imputation, or a natural-history reversion toward the -2.7/yr slope. The company also faces meaningful regulatory uncertainty after the recent AdCom on 7/29/26, and the FDA may still judge the dataset insufficient for approval, which would jeopardize the expected near-term non-dilutive cash infusion of >$230M. Beyond the clinic, litigation with its U.S. commercialization partner, outsourced commercialization control, and the need for additional capital if the launch or approval path slows all raise dilution and execution risk for a company with an early-stage, limited pipeline.
