The case for and against

Updated Oct 14, 2026

Bull case

Cytokinetics is attractive because aficamten has delivered statistically significant and clinically meaningful benefits in nHCM, including improved KCCQ-CSS, pVO2, NYHA class response, NT-proBNP, and broad consistency across prespecified subgroups, which supports a differentiated mechanism and meaningful unmet-need opportunity. Its commercial outlook is also strengthening, with MYQORZO launching strongly in the U.S. and Europe, more than 2,500 REMS-certified HCPs, over 700 prescribers, over 80% of patients on paid therapy, and 2Q26 product sales of about $25MM alongside total revenues of $28.6M. The outlook is further reinforced by a $1.7B balance sheet, advancing regulatory milestones including the November 14, 2026 PDUFA date and a planned 4Q26 sNDA filing, plus a pipeline that could broaden Cytokinetics into a multi-asset cardiology platform.

Bear case

Cytokinetics is still a clinical-stage biopharmaceutical company with no profitable products in the market, so its value depends heavily on uncertain trial outcomes and future regulatory success. While ACACIA met both dual primary endpoints, the business remains exposed to major commercialization and funding risk, especially as Myqorzo must compete in a crowded market and pipeline assets like omecamtiv require substantial spend for comparatively smaller opportunities. Its extensive R&D and SG&A outlays, along with royalty and debt contracts that may limit flexibility, leave the company vulnerable to failed studies, regulatory delays, and an inability to finance ongoing operations.
