The case for and against

Updated Oct 16, 2026

Bull case

Gain Therapeutics is attractive fundamentally because its lead candidate GT-02287 has already shown target engagement, favorable safety and tolerability, and a compelling biomarker response, including an average 81% reduction in CSF glucosylsphingosine after 90 days in patients with elevated baseline levels. The company also has an FDA-authorized Phase 2 path for Parkinson’s disease with or without a GBA1 mutation, and the latest data suggest a multi-biomarker signal—lower DDC, improved or stable MDS-UPDRS scores, and anecdotal gains in smell, gait, and sleep—that supports a disease-modifying thesis rather than purely symptomatic benefit. While Phase 2 initiation has shifted to 1Q27 from 3Q26 and the trial design remains in flux, the extensive preclinical package, ongoing extension data, and platform-driven ability to rapidly identify allosteric modulators strengthen the long-term opportunity.

Bear case

Gain Therapeutics is facing a weak fundamental outlook because the clinical data for GT-02287 do not yet show durable, clearly attributable benefit: the 12-completer MDS-UPDRS II+III mean moved from roughly 2.9 points better at Day 90 to 3.3 points above baseline at Day 270 and only 0.5 points above baseline at Day 360, while medication and deep-brain-stimulation changes were not reported. The safety picture is also not clean, with transient liver enzyme elevations, ALT/AST/GGT increases in the extension chart, and two hospitalized serious adverse events across both study parts, even if both were assessed as unlikely related. Financially, the company had only about $13.1 million in cash at mid-2026 and posted a 2Q26 net loss of $0.11 per share, leaving it dependent on execution, future clinical progress, and potentially dilutive financing.
