The case for and against

Updated Oct 18, 2026

Bull case

OPKO Health is viewed positively because the sale of the remaining BioReference laboratory testing business to LabCorp for $722 million upfront, plus up to $150 million tied to 4Kscore revenues, materially strengthens the balance sheet and simplifies the story. The company ended 2Q:26 with $314.4 million in cash against $349 million in debt, while its pharmaceutical assets and distributor business retain meaningful value through 2027E revenues, NGENLA profit share, and Rayaldee. Its ModeX pipeline adds multiple near- to mid-term catalysts, including Phase I/IIa and Phase I programs such as MDX2001, MDX2003, MDX2004, MDX2301, and MDX3001, which together support an expanding valuation base.

Bear case

OPKO Health is facing a weak fundamental setup after first-quarter revenue of $124.2 million and EPS of -$0.07 both missed expectations, with Diagnostics at $72.2 million, Rayaldee at $6.3 million, and other revenue at $14.0 million all coming in below estimates. The company’s cash, equivalents, and restricted cash of about $341.9 million versus total debt of roughly $345.7 million leaves limited financial flexibility, especially given stated risks that NGENLA, Rayaldee, and BioReference may miss forecasts and that ModeX’s vaccines and antibodies may fail in the clinic. Although management affirmed FY/26 revenue guidance of $530 million to $560 million, the need for a sharp 2H/26 acceleration and the reduced credibility of the upper end make the outlook for execution and earnings quality unattractive.
