The case for and against

Updated Oct 21, 2026

Bull case

AC Immune is attractive fundamentally because its platform spans active immunotherapy, small molecules, and diagnostics across Aβ, Tau, αSyn, and NLRP3, giving it multiple shots on goal in neurodegeneration while addressing areas where simpler administration, lower cost, and longer-lasting responses can be differentiating. The company’s ACI-24 program has shown strong immunogenicity with no ARIA-E events reported, Takeda has already provided a $100M upfront payment, and the agreement still carries up to about $2.1B in potential milestones plus royalties, which materially de-risks the pipeline. Positive PD signals from ACI-7104.056, including biomarker stabilization and encouraging motor readouts, alongside a $26,500 U.S. annual price assumption and 25% PoS with 2031 market entry, support meaningful long-term upside if clinical translation continues.

Bear case

AC Immune is challenged by a clinical-stage profile that still depends on unproven translation from encouraging biomarker signals to durable clinical benefit, especially because the α-syn aggregate/clinical improvement relationship remains less established and the seeding assay is semi-quantitative. While ACI-7104.056 showed week-100 safety, tolerability, and immunogenicity with 100% of treated patients developing antibodies and stronger titers correlating with UPDRS-III stabilization, the study is tiny at n=34 and requires replication in the larger ~250-patient Part 2 before confidence improves. The broader pipeline also appears capital intensive and slow-moving, with FDA meetings expected in early 2027 and ACI-24 requiring a second adjuvant, suggesting responses were below optimal and that meaningful clinical proof may still be years away.
