The case for and against

Updated Oct 15, 2026

Bull case

ADC Therapeutics is viewed positively because ZYNLONTA is already commercially approved, which lowers regulatory risk, supports tangible cash flows, and gives the company a marketed asset with proven physician adoption potential in relapsed or refractory large B-cell lymphoma. The upside thesis is further strengthened by the possibility of broader success across indications, including increased penetration and expansion into additional indolent lymphomas, while the pipeline assets LOTIS-2, LOTIS-5, LOTIS-7, and ADCT-241 provide further optionality across hematologic malignancies and solid tumors. In addition, the capital raise in 4Q25 is expected to extend cash runway until 2028, giving ADCT sufficient balance-sheet support to pursue expansion into earlier lines of treatment and sustain execution.

Bear case

ADC Therapeutics is facing a difficult fundamental setup because its only marketed drug, ZYNLONTA, delivered just $18.6M in 2Q net sales, while management still relies on uncertain expansion into 2L+ DLBCL to revive growth. The biggest overhang is LOTIS-5, where ZYNLONTA plus rituximab showed only modest PFS improvement with no OS benefit and a materially higher Grade 5 TEAE rate of 13.2% versus 4.6%, prompting FDA “substantial concerns” about the benefit-risk profile. With regulatory risk elevated, a crowded DLBCL landscape, and a limited pipeline beyond NHL, the company’s commercialization and valuation case appear fragile.
