The case for and against

Updated Oct 15, 2026

Bull case

Ardelyx is supported by a differentiated tenapanor franchise that remains the only sodium transport inhibitor approved for IBS-C and phosphate reduction in dialysis CKD, giving it two large markets with substantial unmet need and limited near-term competition. Despite 2Q26 reimbursement headwinds and an unexpected leadership transition, Ibsrela still grew 33% y/y to $86.2M while Xphozah rose 27% y/y to $31.9M, and management reaffirmed $110M-$120M of FY26 Xphozah revenue with a path to profitability in 2027. The company’s outlook is further strengthened by >95% gross margins, $281.8M in cash and equivalents at the end of 2Q26, an expanded sales and reimbursement effort, and optionality from CIC and broader IBS-C growth opportunities.

Bear case

Ardelyx is exposed to meaningful regulatory risk because its accelerated approval strategy could still require additional data, longer studies, or narrower labels that weaken commercial traction. Its outlook is further pressured by reimbursement uncertainty, since changes in pricing structures could reduce access and revenue potential for IBSRELA and XPHOZAH. Commercial execution risk remains high, and intensifying competition from generic players and established companies such as AMGN, OPKO, Shield Therapeutics, and AKBA could limit adoption and make sustained growth difficult.
