The case for and against

Updated Oct 22, 2026

Bull case

Braveheart Bio is supported by a compelling clinical and commercial setup for BHB-1893, whose phase 2 data showed ~5.5-6 point placebo-adjusted KCCQ gains at Week 12, plus improvements in pVO₂, diastolic relaxation, and early LV remodeling signals. The company’s cash from the IPO raised about $440M appears sufficient to fund both global phase 3 programs through completion into ~2029, reducing near-term financing and dilution risk while several de-risking catalysts arrive in 1H27 and 2H27. Fundamentally, the bullish case rests on nHCM differentiation in a whitespace market and on oHCM share gains through a simpler one-step titration profile that could make a validated drug class easier to use and commercially attractive.

Bear case

Braveheart Bio is a high-risk clinical-stage biotech because its lead asset, BHB-1893, has only shown encouraging KCCQ and remodeling signals in a relatively small 12-week phase 2 study, leaving the global phase 3 NOBLEHEART-HCM trial to prove the effect is durable and scalable. The data also show meaningful variability concerns, with about 35% of ODYSSEY patients needing down-titration and/or discontinuation, including 22% due to LVEF drops, while four nHCM patients in phase 2 had shallow LVEF reductions. Commercially, the bigger threat is that if BHB-1893 only looks comparable to Afi in nHCM, market entry will be tougher because competing CMIs may already be established and morphology-specific response remains uncertain.
