The case for and against

Updated Oct 21, 2026

Bull case

Compass Therapeutics is supported by a compelling clinical signal in 2L BTC, where tovecimig plus paclitaxel extended median PFS to 4.7 months versus 2.6 months for paclitaxel alone, with a hazard ratio of 0.44 and a 56% reduction in progression risk. Despite the non-significant ITT OS readout, the analyst views it as difficult to interpret because 54% of control-arm patients crossed over to tovecimig, while crossover patients showed median OS of 12.8 versus 6.1 months in a non-prespecified subset. The company also has a $179.9MM cash position, expected CTX-8371 and CTX-10726 data in 4Q26, and a sizeable platform opportunity in the multi-billion dollar PD-1 x VEGF market.

Bear case

Compass Therapeutics is facing a materially more uncertain regulatory path for tovecimig after the FDA denied an in-person meeting and indicated a second study showing an overall survival benefit would be needed for approval. The company’s inability to obtain clear guidance, combined with only written correspondence and no further clarification, raises approval risk and led the analyst to remove all tovecimig revenue from the model despite a planned 1H27 BLA filing and possible 1H28 ODAC review. Fundamentally, the stock also carries high clinical, competitive, and financing risk, with valuation now tied mainly to a single lead asset and assumptions further pressured by extended timelines, a higher fully diluted share count, and potential dilutive capital raises.
