The case for and against

Updated Oct 23, 2026

Bull case

Cardiol Therapeutics is supported by a credible clinical thesis in recurrent pericarditis, where the peer-reviewed MAvERIC-Pilot showed rapid, durable symptom relief, a 3.7-point pain reduction at Week 8, and an annualized event rate falling from 5.8 to 0.9 during study participation. The ongoing Phase III MAVERIC trial reached its original 110-patient target and will expand to up to 150 patients after a lower-than-expected blinded event rate, with topline results pushed to mid-2027, but published rilonacept-withdrawal data showing ~75–82% recurrence suggests the low event rate may reflect CardiolRx efficacy rather than weak trial activity. Its oral, steroid-sparing profile could position it ahead of injectable IL-1 blockers or as a step-down therapy after biologics, while CRD-38 adds another optionality layer in inflammatory heart disease and heart failure.

Bear case

Cardiol Therapeutics is facing a fundamentally weak risk-reward profile because its valuation still depends on clinical proof that remains incomplete, with blinded MAVERIC data unable to distinguish whether any apparent event shortfall comes from CardiolRx or placebo. Although CRP fell from 5.7 mg/dL to 0.3 mg/dL in a ten-patient subgroup, the evidence base remains small and the company is still assigning only 70% success in recurrent pericarditis, with just 20% and 10% for acute myocarditis and heart failure. Its cash only supports operations into 3Q27, while financing risk, commercial execution risk, and stock-price proximity to $1 create meaningful dilution and delisting concerns.
