The case for and against

Updated Oct 18, 2026

Bull case

  • Delcath Systems is supported by accelerating commercial execution, with 2Q26 revenue of $29.1M beating consensus and Hepzato KIT sales of $27.1M driven by 30% year-over-year volume growth, while gross margin expanded to 89.8% and adj. EPS reached $0.07.
  • Its network buildout is also improving the quality of demand, as active treatment centers rose to 31 with a target of 37 by year-end, and roughly 80% of active centers now have NCI Comprehensive designation, which should deepen referrals and lift volume per site.
  • Management’s confidence in 2026 is reinforced by steady new patient starts at roughly 0.5 per site per month, backup teams at several high-volume centers to reduce seasonal disruption, and raised revenue guidance of $104M-$108M alongside expectations for positive adj. EBITDA and strong cash generation.

Bear case

Delcath Systems is a pure-play standalone interventional oncology company, but its fundamentals remain fragile because the business is still highly dependent on the pace of U.S. commercialization for Hepzato Kit and on sustained traction from new commercial site activations. Its outlook is further pressured by meaningful execution risks around pricing, reimbursement, competition, supply constraints, tariffs, and access to financing, any of which could slow adoption and strain operating flexibility. While valuation context from BTG at about 5x sales in 2019 and Sirtex at about 6.7x sales in 2018 suggests the category can command strategic multiples, that support only matters if Delcath can consistently convert clinical progress into durable revenue growth.
