The case for and against

Updated Oct 21, 2026

Bull case

Establishment Labs Hldgs is supported by accelerating U.S. Motiva adoption, with Q2 revenue of $67.5M and U.S. revenue rising to $24.7M, more than doubling from the prior-year period as the company surpassed 2,000 U.S. accounts and 100,000 Motiva implants since launch. The firm also posted its fourth consecutive quarter of positive adjusted EBITDA at $3.7M, raised 2026 revenue guidance to $269M-$271M, and is expected to reach free cash flow positive in the second half of 2026 and full-year 2027. Beyond augmentation, Preservé, Flora, and the pending FDA reconstruction label expansion add multi-year upside, while 2026 revenue is still modeled around 28% growth even before any reconstruction approval.

Bear case

Establishment Labs Hldgs is weighed down by poor Motiva implant sales, intensified global competition from better capitalized rivals, and the risk that it cannot secure additional regulatory clearances or launch products on time in emerging and developed markets. Its capital outlook also looks fragile because profitability targets have been delayed, supply depends on a single source of medical-grade silicone, and management faces execution risk in attracting and retaining executives and sales staff. Although outside the U.S. revenue rose 4.4% y/y and Europe direct markets grew 16%, ongoing weakness in the Middle East and broader sensitivity to pricing, foreign exchange, and economic uncertainty in emerging markets support a negative fundamental view.
