The case for and against

Updated Oct 15, 2026

Bull case

Edgewise Therapeutics is viewed positively because EDG-7500 has shown broad Phase 2 efficacy in both oHCM and nHCM, including roughly 55% resting LVOT-G reduction, about 65% NT-proBNP reduction in nHCM, and meaningful gains in KCCQ and NYHA class, while preserving LVEF and avoiding heart-failure events across 53 patients. The case is further strengthened by differentiation potential versus cardiac myosin inhibitors through simpler monitoring, possible REMS-free use, and a favorable safety profile across 240+ patients and 700+ echocardiograms. Financially, the Servier transaction delivered $1.55 billion upfront and up to $2.65 billion total consideration, leaving approximately $2.05 billion pro forma cash and zero debt, which should fund Phase 3 development and reduce dilution risk.

Bear case

Edgewise Therapeutics is a high-risk, precommercial biotech whose investment case depends almost entirely on EDG-7500, yet the dataset remains small at 53 patients over 12 weeks and does not yet prove that Phase 3 will replicate the reported LVOT-gradient, NT-proBNP, and KCCQ benefits. The negative outlook is driven by the possibility that FDA requires a REMS or echo-monitoring cadence at EOP2, which would erase the hoped-for operational differentiation, while safety concerns around systolic dysfunction, atrial arrhythmias, and exposure-linked LVEF effects remain central class risks. Financially, the company has no commercial products and could face meaningful funding pressure if the Servier transaction stalls or if Phase 3 and manufacturing costs rise, with even the downside case implying the stock could trade near cash, about $10.
