The case for and against

Updated Oct 14, 2026

Bull case

Structure Therapeutics is attractive fundamentally because aleniglipron is showing durable efficacy and a cleaner safety profile, with up to 16% weight loss at 72 weeks, an 88% overall completion rate in the OLE, and lower nausea, vomiting, and discontinuation rates after slower titration. The start of the Phase 3 ACCOMPLISH trial adds a major clinical milestone, while additional Phase 2 readouts in 2H26 could reinforce the view that the asset can combine meaningful weight loss with improved tolerability and favorable liver safety. Its oral amylin program also looks promising, as ACCG-2671 showed up to 3% weight loss after a single dose, target engagement, dose-proportional PK, and a six-day half-life that supports weekly dosing.

Bear case

Structure Therapeutics is a clinical-stage company whose lead asset, aleniglipron, still faces major execution risk because Phase 2b gains in obesity and diabetes must be reproduced in larger Phase 3 studies before approval. It also operates in a crowded GLP-1 market with more than 20 competitors, including entrenched big pharma products, so aleniglipron needs a clearly differentiated profile and may still require a commercial partner to win share. Financially, the company is expected to keep posting net losses until commercialization and likely through the early launch phase, while additional capital raises could dilute shareholders and the model’s value assumptions remain highly sensitive to clinical, regulatory, and penetration outcomes.
