The case for and against

Updated Oct 16, 2026

Bull case

InnovAge Holding is attractive fundamentally because its PACE model addresses two structural healthcare problems—rising costs and poor outcomes—while benefiting from strong evidence cited by HHS and early discussions with CMS that could expand the program over time. The company just delivered a strong F4Q26 with EBITDA of $24.3M, up 114% year over year and 33% above estimates, on revenues of $262.0M, up 18%, driven by 6.9% census growth, 10% revenue PMPM growth, and only 0.3% external cost PMPM growth from lower SNF utilization and pharmacy costs. Looking ahead, FY27 guidance of $105M-$115M EBITDA and 8,625-8,850 census suggests margin expansion toward 10.3% at the midpoint, supported by member-experience investments, AI-enabled operations, and further operating leverage from a turnaround that appears complete.

Bear case

InnovAge Holding is vulnerable because roughly 55% of revenue depends on Medicaid, exposing it to state budget pressure, OBBB-related funding uncertainty, and the risk of sanctions on new member enrollment under Medicare and Medicaid regulation. The company also faces execution risk from the transition to the v28 risk-adjustment model, which could pressure reimbursement despite the offset from dementia coding, especially if profitability in at-risk contracts weakens. Although the PACE model has transparent value delivery and HHS cited strong evidence of lower hospitalizations, ED visits, and mortality, the stock still faces meaningful downside if reimbursement tightens or a public health emergency disrupts operations.
