The case for and against

Updated Oct 14, 2026

Bull case

Opus Genetics is supported by a sharper regulatory and clinical path for OPGx-BEST1 after the FDA aligned on a randomized, sham-controlled pivotal design with a more achievable responder threshold of mean microperimetry improvement of at least 3 dB across five loci plus a patient-reported outcome. The company’s outlook is strengthened by Cohort 2’s higher-dose, over-enrolled study in 2026, with dosing completion in 4Q26 and 3-month top-line data expected in 2Q27, while current cash burn is said to extend into 2029. Beyond BEST1, OPGx-LCA5 is the most advanced registrational opportunity with dosing planned to begin in 4Q26, and four clinical readouts expected in 2027 across BEST1, LCA5, RDH12, and MERTK could validate a broader IRD platform.

Bear case

Opus Genetics is a highly speculative gene-therapy story whose value depends heavily on two lead programs, OPGx-LCA5 and OPGx-BEST1, both still exposed to clinical, regulatory, and commercialization risk. Although FDA alignment and the five-patient Cohort 1 signal for OPGx-BEST1 are encouraging, the data remain small, open-label, and uncontrolled, while OPGx-LCA5 has completed enrollment in its six-month run-in period but has not yet begun treatment dosing. With only one partnered, revenue-generating product, significant losses since inception, and a need for substantial capital despite a cash runway expected into 2029, dilution, delays, or weak efficacy could materially pressure the business.
