The case for and against

Updated Oct 14, 2026

Bull case

Kestra Medical Techs is benefiting from a powerful combination of WCD market expansion, share gains against the incumbent, and commercial execution that is lifting both prescription volumes and revenue conversion through a higher in-network mix and better revenue cycle management. F1Q27 sales rose to $31.0M, up 60% y/y and about $2M above consensus, while gross margin expanded to 56.5% from 45.7% a year earlier, reinforcing the view that the business can scale with improving unit economics. The positive outlook is further supported by an estimated ~15% U.S. market share, approximately $320M of liquidity, and FY27 revenue guidance raised to $141M, suggesting durable momentum despite continued investment in sales, R&D, and infrastructure.

Bear case

Kestra Medical Techs is still in an early commercial ramp, but its valuation already reflects rich expectations at 7.4x forward sales, about 140% above the small-cap MedTech average, leaving limited room for disappointment. Although revenue has nearly doubled to $28.6M and the company has ~15% unit share, it faces meaningful execution, reimbursement, and balance-sheet risks while competing in a duopoly against a far larger incumbent. Patient selection remains difficult in the WCD market, and despite strong wear-time and low false-alarm data, a current US-only footprint, lower revenue per fitting of about $5,400, and reliance on payer mix could constrain profitability and make growth uneven.
