The case for and against

Updated Oct 14, 2026

Bull case

Karyopharm Therapeutics is viewed positively because selinexor has shown unusually strong biomarker-driven efficacy in TP53 wild-type endometrial cancer, where SIENDO subgroup data showed a 65% reduction in progression or death and mPFS of 39.5 months versus 4.9 months on placebo. The upcoming Phase III XPORT-EC-042 readout in mid’26 is a major catalyst, with roughly 220 patients in the mITT analysis and 276 in the ITT population, and a positive result could meaningfully expand XPOVIO’s label and revenue base; U.S. net revenues were $114.9 million in 2025 and are guided at $115-130 million in 2026. In myelofibrosis, SENTRY added further upside by meeting SVR35 at 49.8% versus 28% on ruxolitinib and showing an OS benefit at ~12 months, while cash into 3Q:26 reduces near-term financing risk.

Bear case

Karyopharm Therapeutics is under pressure because the FDA withdrew XPOVIO’s accelerated approval in 3L+ DLBCL after the company could not complete the confirmatory trial amid an evolving treatment landscape, forcing it to stop further development in that indication. The setback removes a revenue stream, while selinexor still faces meaningful execution risk in relapsed/refractory multiple myeloma, myelofibrosis, and endometrial cancer, including a missed Abs-TSS co-primary endpoint in the Phase III SENTRY trial and uncertainty in XPORT-EC-042. Financially, the company carries outstanding debt, will likely need additional capital, and faces long-term dilution risk despite a $457M total projected market value under a DCF approach.
