The case for and against

Updated Oct 14, 2026

Bull case

Lantheus Holdings is well positioned because its core franchise remains resilient: 1Q26 revenue of $377.3 million beat expectations by about 6%, with PYLARIFY at $240.9 million, Neuraceq at $35.4 million, and DEFINITY plus partnerships also contributing to a clean upside surprise. Management also showed improving profitability, as gross margin reached 67.0% and adjusted operating margin was 34.2%, while the company continues to benefit from favorable product mix and leverage across operating expenses. The outlook is further supported by multiple catalysts, including FDA approval for PYLARIFY TruVu in March, tentative approval for PNT2003, and a de-risked LNTH-2501 profile after the FDA raised only third-party manufacturing issues, not safety or efficacy concerns.

Bear case

Lantheus Holdings is exposed to heavy concentration risk because PYLARIFY and DEFINITY accounted for over 80% of total sales in 2022, so any slowdown in adoption, pricing pressure, or supply disruption could materially weaken its financial profile. The business also depends on a fragile just-in-time radiopharmaceutical supply chain tied to cyclotron networks and radioisotope decay, making execution vulnerable to manufacturing, logistics, and network interruptions. In addition, reimbursement and regulatory pressure from Medicare and third-party payors, plus competition and integration risks from recent deals and leadership changes, could limit demand and cause operational shortfalls.
