The case for and against

Updated Oct 14, 2026

Bull case

Omeros is viewed favorably because YARTEMLEA’s early U.S. launch has already shown real commercial traction, with $28.5M of 2Q sales, 74 unique ordering accounts by 2Q, and about 75% of orders coming from adults, suggesting broadening adoption in a larger market than pediatric-first launches. The company’s outlook improves further as the permanent J-code went live July 1 and NTAP begins October 1, both of which should reduce reimbursement friction and support a launch that is already working before those tailwinds fully arrive. The thesis is also supported by physician feedback implying more than 30% market share over time and peak sales around $400M, while the EU remains upside and the Novo Nordisk partnership adds non-dilutive milestone and royalty value.

Bear case

Omeros is viewed negatively because its near-term fate remains highly dependent on narsoplimab, while development, regulatory, and commercialization setbacks could materially impair value if approval or sales disappoint. The European CHMP negative opinion in June and the ongoing re-examination underscore regulatory risk, and the limited eligible TA-TMA population plus competition from larger, better-resourced companies further constrain the addressable market and execution outlook. Even with the U.S. YARTEMLEA launch and NVO partnership, the investment case is still vulnerable to pipeline and clinical trial uncertainty, with a 12.5% WACC and 12.0x exit multiple reflecting that risk.
