The case for and against

Updated Oct 15, 2026

Bull case

OptimizeRx is attractive because its core point-of-care network reaches more than 2M HCPs and 240M patients, while DAAP and Micro-Neighborhood Targeting deepen differentiation and support higher-margin, AI-driven monetization. Even after revenue guidance was cut to $95M-$100M and 2Q26 revenue fell 30% Y/Y to $20.5M, EBITDA margin expanded to 24.1% and EBITDA guidance stayed at $21M-$25M, showing strong cost discipline and operating leverage as the mix shifts away from lower-margin managed services. The balance sheet also improved materially after the May 2026 refinance into a $35M facility at about 6.5%, net cash turned positive at $5.1M, and DeepIntent’s May 2026 EHR integration and expected 4Q revenue ramp add a second growth engine.

Bear case

OptimizeRx is facing a deteriorating fundamental setup because 2Q26 revenue fell to $20.5M, down 30% Y/Y, after the loss of a low-margin managed-service stream and a zero-revenue customer, while net revenue retention dropped below 100% for the first time in 2Q26 as large accounts optimized spend. The company remains highly exposed to concentration risk, with roughly 62% of 2025 revenue tied to just two eRx/EHR channel partners and 54% of TTM revenue coming from top 20 pharma manufacturers, leaving execution, partner bargaining power, and customer churn as persistent threats. Although gross margin improved to 76.5% and EBITDA margin expanded to 24.1%, the 2026 outlook still depends on a steep 4Q ramp to 35%–40% of annual sales amid MFN, IRA, and broader pharma budget uncertainty.
