The case for and against

Updated Oct 16, 2026

Bull case

Protagonist Therapeutics is attractive because two FDA approvals have validated its peptide-discovery platform while generating royalty and milestone income that reduces financing risk and supports a self-funded pipeline. Icotrokinra’s strong launch, with over 18,000 prescriptions and about 11,000 patients treated by 7,000 writers, plus the possibility of $8.3B peak PsO sales and $16.8B combined across PsO, PsA, and IBD, suggests a large, underappreciated royalty stream. Rusfertide adds a second royalty source with 14% to 29% worldwide royalties and roughly $700M in peak royalties at $2.9B sales, while $849.5M in cash, $275M in approval-related payments, and a $300M buyback strengthen shareholder returns.

Bear case

Protagonist Therapeutics is viewed negatively because its valuation depends heavily on partnered commercial programs, making execution by Johnson & Johnson and Takeda a key risk while any setback in sales, reimbursement, or uptake could quickly pressure fundamentals. Its wholly owned pipeline adds further uncertainty, since promising early data may not translate into later-stage success, regulatory decisions remain unpredictable, and competition in I&I and oral IL-23R therapies could limit Icotyde and future assets. Financially, higher-than-expected spending on the wholly owned pipeline, combined with standard clinical, manufacturing, funding, and commercial risks, leaves the company vulnerable to uneven development outcomes and weaker returns.
