The case for and against

Updated Oct 21, 2026

Bull case

Sarepta Therapeutics is viewed positively because Elevidys continues to show clinically meaningful differentiation, with ENDURE interim data indicating no treatment-related ALI or immunosuppression-related TEAEs in 11 prophylactic sirolimus patients, versus ALI rates of 17% in ambulatory and 33% in non-ambulatory patients without prophylaxis. The exploratory PRO analysis also supports the franchise’s mobility benefit, as EMBARK Part 2 patients showed slower PROMIS mobility declines and a median time to first meaningful improvement of 1 year versus no response in controls. Fundamentally, the outlook is strengthened by reduced legal uncertainty after the $39M REGENX/UPenn patent settlement and by upcoming 1Q:27 safety and expression data from about 25 non-ambulatory patients.

Bear case

Sarepta Therapeutics is viewed negatively because the ELEVIDYS evidence package remains too weak to justify stronger sales assumptions: the 3.32-point NSAA advantage came from 25 older patients versus 99 external controls, while the toddler data lacked functional outcomes and EMBARK’s placebo arm improved more at Year 1 than the comparator cohort. Commercial momentum is already deteriorating, with 2Q26 ELEVIDYS revenue of $98.1M declining 3.8% quarter over quarter, and the company still faces meaningful toxicity concerns including 7-9% serious acute liver injury and about 7% hospitalization rates. Beyond ELEVIDYS, the broader DMD franchise is clouded by regulatory and competitive risk across the PMO portfolio, and the limited sirolimus dataset in only 11 patients does little to remove uncertainty around future uptake or debt-service capacity.
