The case for and against

Updated Oct 21, 2026

Bull case

Telix Pharmaceuticals is viewed positively because its core Illuccix franchise is being augmented by multiple high-value growth drivers, including Pixclara’s FDA approval for glioma imaging and ITM-11’s strong COMPETE data, where median PFS improved to 23.9 months versus 14.1 months with everolimus and grade 3/4 treatment-related adverse events were lower at 18% versus 40%. The company’s isotope supply platform adds a scalable, profitable revenue stream, generating $273MM in 2025 revenue with ~40% CAGR from 2021–2025 and reaching >65 countries, while also reducing dependence on external suppliers for future commercial demand. Financially, ITM produced $156MM of revenue in 1H26 with manufacturing gross margins above 40%, expected annualized 2026 EBITDA of about $106MM, and pro forma cash of roughly $325MM as of June 30, 2026, supporting broader pipeline investment.

Bear case

Telix Pharmaceuticals is exposed to substantial fundamental risk because its valuation still depends heavily on speculative pipeline assets, with TLX591/TLX592 modeled at only 10%, 35%, and 65% approval probabilities across key indications. Although Illuccix and Gozellix provide current revenue support, the business remains vulnerable to an inability to meet sales estimates, while Zircaix and other programs still face regulatory and clinical uncertainty. The stock also carries competitive pressure, the possibility of delays in registrational programs, and long-term dilution risk, making the investment case dependent on execution that has yet to be proven.
