The case for and against

Updated Oct 21, 2026

Bull case

Viridian Therapeutics is supported by a differentiated IGF-1R platform that has shown marked improvement in proptosis, diplopia, and CAS versus teprotumumab, with management and KOLs viewing veligrotug as a best-in-class option for active and chronic TED. Early Lumvoa launch data are encouraging, with 95% of the ~2,000 core prescribers contacted, first doses administered in July, and multiple KOLs already writing prescriptions, while payer friction appears manageable and could improve further with a permanent J-code expected in 1Q27. The opportunity is reinforced by a large, underpenetrated TED market, where Tepezza generated $576m in Q2 and chronic TED remains less than 5% treated, while elegant subcutaneous options and a broader pipeline add longer-term growth optionality.

Bear case

Viridian Therapeutics is a high-risk, single-asset-driven story whose lead TED programs must overcome regulatory and safety uncertainty while facing an entrenched competitor, Tepezza, with a 6-7 year head start and the backing of Amgen’s resources. The KOL feedback points to real commercial friction: active TED patients are only 10–25% of TED volume, anti-IGF-1R use is constrained by hearing-loss fears, recurrence concerns, and inconsistent prescribing, with utilization ranging from 50% to 10% and some doctors sharply reducing use. Financially, the company is still burning cash as a clinical-stage biopharma, posting 2Q26 R&D of $72m, SG&A of $55m, and a net loss of $127m, while management may need additional capital and a failure of veligrotug could leave the stock near net cash value.
