The case for and against

Updated Oct 17, 2026

Bull case

vTv Therapeutics is viewed favorably because cadisegliatin addresses a major unmet need in type 1 diabetes: persistent hypoglycemia despite modern insulin delivery, with recent data showing this burden remains in about 54% of AID users. The lead Phase 3 CATT1 trial is fully enrolled with 166 patients randomized, is powered at 90% to detect a 30% reduction in Level 2/3 hypoglycemia, and has topline data expected in mid-2027, while prior SimpliciT1 results showed statistically significant HbA1c lowering and a 50% reduction in symptomatic hypoglycemia without a DKA signal. With FDA Breakthrough Therapy Designation, favorable KOL feedback, and a projected $840M peak sales opportunity, the company has a compelling late-stage clinical and commercial setup.

Bear case

vTv Therapeutics is fundamentally challenged by a high-risk, single-asset-heavy pipeline, where cadisegliatin’s commercial case still depends on proving both clinical benefit and regulatory acceptance in type 1 diabetes despite the persistent hypoglycemia burden in AID users. The company also faces meaningful execution and financial pressure, with 2Q26 operating expenses of $13.9M, 2Q26 EPS of ($1.05), and projected 2026 operating expenses of $53M, while cash and equivalents of $86.6M only support operations into 2028. Even if development succeeds, stiff T1D competition and reliance on third-party manufacturing add further uncertainty, making the risk/reward profile unattractive.
