The case for and against

Updated Oct 15, 2026

Bull case

Zevra Therapeutics is attractive because its U.S. MIPLYFFA launch is proving durable, with 184 cumulative prescription enrollment forms through 2Q26, 14 new starts in 2Q26, and $30.2MM in U.S. net sales, while payor access remains stable at 69% of covered lives. The company’s commercial strategy is increasingly effective at finding previously undiagnosed NPC patients across pediatric and adult segments, supporting a longer runway than the currently diagnosed 300–350 U.S. patient pool and the broader ~900-patient estimate suggests. With about $260M of cash and investments, no debt, 2041 patent protection, and optionality in Europe and celiprolol, the company has both financial flexibility and multiple pathways to incremental value creation.

Bear case

Zevra Therapeutics is viewed negatively because its revenue outlook is being pressured by volatility in the timing and size of non-French EAP reimbursements, which has forced lower near-term ROW/EAP estimates despite commercialization progress for Miplyffa. The stock also faces meaningful execution risk from a recent competitor approval, possible payer preference for that rival, and emerging safety or tolerability issues that could slow launch ramp, raise discontinuations, and reduce peak sales potential. Fundamentally, the company remains exposed to clinical, regulatory, litigation, and balance-sheet risk, with a history of losses and a likely need to raise additional capital to support its long-term commercial strategy.
