The case for and against

Updated Oct 15, 2026

Bull case

IQVIA Holdings is well positioned by its diversified exposure to both clinical development and commercialization, with Commercial Solutions benefiting from record novel drug approvals and R&DS showing stronger bookings, a record NTM backlog revenue of $9.2B, and improving RFP flows. The 20 novel drug approvals in 3Q26, the highest since 2021 and over 3 standard deviations above the five-year quarterly average of about 11.9, should translate into a multi-year revenue tailwind given the 3–6 month revenue lag and roughly 50% of commercialization spend concentrated in the first two years after approval. Management’s commentary on healthier large-pharma and biotech demand, a refocused down-market biotech strategy, and early margin expansion supports durable earnings growth and reinforces the company’s strong execution profile.

Bear case

IQVIA Holdings is challenged by execution risk in its core CRO business, where 50% to 60% of clinical trials are delayed by slow patient enrollment, site selection errors, or drug-related issues, and any missed timelines could damage customer trust and future demand. Its scale in data and analytics is a strength, with 1.2 billion longitudinal patient records, ~64 petabytes of proprietary data, and ~120 billion healthcare records annually, but that advantage also heightens exposure to cybersecurity failures or data loss that could impair its reputation as a trusted custodian. Financially, interest expense is likely moving higher in 2027, with estimates in the low-to-mid $800M range, while funding delays, weaker pharma spending, FX volatility, and debt-service risks add pressure to earnings visibility and balance-sheet flexibility.
