The case for and against

Updated Oct 14, 2026

Bull case

Praxis Precision Medicine is supported by a diversified late-stage pipeline that gives it multiple shots at meaningful clinical and commercial value, including ulixacaltamide in essential tremor, relutrigine in DEEs, and additional programs across movement disorders and epilepsy. The ulixacaltamide data are especially encouraging, with Study 1 in 473 patients showing mADL11 improvement of 4.3 points versus 1.7 on placebo at Day 56 and Study 2 showing 55% of stable responders maintained response versus 33% on placebo, reinforcing both efficacy and durability. Relutrigine further strengthens the thesis, with 46% and 53% placebo-adjusted seizure reduction in EMBOLD, a December 27, 2026 PDUFA for SCN2A and SCN8A DEEs, and EMERALD topline data expected in 4Q26 that could materially broaden its addressable market.

Bear case

Praxis Precision Medicine is viewed negatively because its thesis still hinges on clinical-stage assets with no approved products, leaving valuation exposed to binary trial outcomes, regulatory uncertainty, and execution risk. Competition is also intensifying in ET, epilepsy, and DEE, with other therapies already ahead in development, while dependence on third-party suppliers adds another layer of delay risk. Even where early signals exist, expectations for only a 20-30% placebo-adjusted reduction and possible deterioration in larger studies suggest limited efficacy durability, weakening the fundamental case.
